There is a non-refundable processing fee of $250 to open a pre-payment account that will go toward the surgery fee. If the account is closed without the patient having surgery, our office will retain the $250 processing fee. To utilize the Pre Payment Account option complete the Pre-Payment Form and mail it with your cashier’s check or money order made payable to Chapel Hill Tubal Reversal Center to:

Once you have opened a pre-payment account, you may send additional payments of any amount greater than $10 as you are able. Each time you send a payment, complete the Pre-Payment Form, print it, and mail it with your payment. You may receive the reduced fee while using the pre-payment account option, but the balance must be paid in full prior to scheduling a surgery date. For the standard fee, you do not have to wait until the balance is paid to schedule surgery, but the balance will be due in full at three weeks prior to the scheduled surgery date. Accounts are considered inactive if payments are not made for a period of 12 months or more. In this case, patients are no longer locked into the fees that were in place at the time they began the account and current fees will apply.
